All people deserve access to better health, so we’re committed to helping community members across a range of non-medical needs vital to achieving and maintaining good health.

In addition to help with navigating and accessing medical care, we provide job training, legal services, youth support, and support for individuals transitioning out of incarceration.

Health Insurance Enrollment

Certified Assistors are available to enroll you and your family in low-cost, quality health insurance available through the Affordable Care Act. They will help you compare options and apply for assistance that could lower your costs, including Medicaid or Child Health Plus. Anyone can apply at any of our clinics.

HIV Care & Support

Care Coordination

For HIV-positive individuals, case managers help coordinate across medical and behavioral health services, empowering clients with resources, education, and the tools they need to become their own best advocates.

Adult Day Health Services

Our Adult Day Health Care (ADHC) program is for members (18+) of our community who are living with HIV/AIDS or who are at heightened risk for HIV infection and eligible for Medicaid.

Instead of treating specific ailments, we support clients with an integrated, whole-body approach to a lifetime of healing through long-term support, therapy, in-house referrals to healing services, and access to supportive case management. Day healthcare services include:

  • Individual and group services
  • Supportive case management
  • Experienced social workers and mental health counselors
  • Medication support
  • Transgender services and support groups
  • Nutritional support
  • Foot reflexology
  • Creative art therapy
  • Recreational activities
  • One hot meal on the day of services

Youth & Prevention Services (YPS)

Our caring team of providers offers compassionate, judgment-free healthcare to the youngest members of the community. Through access to routine care and education, we aim to lay the groundwork for a lifetime of positive health. Our Youth & Prevention Services team specializes in serving LGBTQ+ young people, trans people of color of any age, active substance users, and young people who are experiencing homelessness or who are unstably housed.

Housing Works YPS:

  • Offers 4th generation rapid HIV Testing and Hep C Testing at no cost
  • Provides care coordination, healthcare, and supportive services in order to make housing and healthcare accessible to all
  • Connects HIV-positive and HIV-negative individuals with lifesaving medical treatment, such as PrEP, PeP, and HIV treatment
  • Promotes open communication about sexual and general health through our drop-in center
  • Provides evidence-based interventions designed to address risky behavior and promote healthy choices for MSM and transgender individuals of color
  • Supports individuals struggling with substance use in a judgment-free environment
  • Assists transgender individuals with safe transition practices, including hormone replacement therapy (HRT) and guidance on gender reassignment surgery, including assistance with name change from our legal department

Justice Initiatives

To promote stronger, safer communities, we provide specialized wraparound services to support successful reentry for individuals involved in the criminal legal system.

Reentry Program

For individuals 18 and older, before and after their release from NYC, NYS, or a Federal correctional facility in the past 2 years, services include:

  • Case management
  • Connection to healthcare
  • Vocation counseling and job training
  • Placement assistance and employment retention counseling

Cannabis Employment Assistance & Training Experience (CREATE)

CREATE is a 6-week paid training program for individuals with prior involvement in the criminal legal system in the past three years to create opportunities for employment in the NYC cannabis retail industry. The program combines two weeks of classroom training with four weeks of internship placement at a licensed dispensary. Through this combination of classroom education and practical application, the program equips participants with the knowledge, skills, and professional customer service abilities to enter the sector, and provides ongoing support to promote employment stability and career advancement.

Alternative To Incarceration (ATI)

ATI provides court-mandated, comprehensive wraparound services for individuals 18 and older charged with misdemeanors, non-violent felonies, or violent felonies through Manhattan and Brooklyn criminal and supreme courts. Services include:

  • Case management
  • Individual & group counseling
  • Toxicology screenings
  • Court advocacy & peer support
  • Gun diversion services to reduce violence in NYC communities

Emergency and Transitional Housing (ETH)

ETH provides 12 months of transitional housing for individuals 18 and older experiencing homelessness or housing instability who (1) have been released from incarceration in the past 12 months or (2) are actively enrolled in pre-trial or community supervision. Onsite services include:

  • Case management
  • Housing placement assistance
  • Primary medical care
  • Psychiatric & behavioral healthcare
  • Vocational support

Job Training

Our Job Training Program provides real-world training for real-life jobs within a community that fosters healing and support. Housing Works Community Healthcare guarantees full-time employment and living wages for all graduates so that stable pay and sustained health are always in sight.

Legal Services

All clients enrolled in Housing Works Community Healthcare’s case management services are eligible for free legal services. Our legal experts can help you with housing, public assistance, family law, wills, and consumer laws. We also take on cases that are important to the HIV-Positive community and have the power to change precedents for all.

A Note About Our Funding

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
